If you would like to register please complete a “New Patient Form and Questionnaire”. You will be asked to provide proof of ID (passport, ID card) and proof of your address (utility bill, bank statement etc). Please check with reception if you are unsure whether you reside within our practice area. For clinical safety reasons we are unable to register patients who live outside the practice boundary. If you are on regular medication or have a history of any serious illness the receptionist will arrange an appointment for you with the doctor or nurse so that we can record the necessary details about your medical problems.
All our patients are allocated a named GP. This means that you have a doctor who has overall responsibility for the care and support that the practice provides to you. This does not prevent you from seeing any GP in the practice at any time should you chose to. You will be told your named GP once you have fully registered.

PLEASE DOWNLOAD ALL THE FORMS AND ONCE COMPLETED HAND THEM IN AT RECEPTION ALONG WITH YOUR PROOF OF ID AND RESIDENCY. ALTERNATIVELY YOU CAN OBTAIN COPIES OF THE REGISTRATION FORMS FROM THE RECEPTION DESK. FORMS CAN ALSO BE SCANNED AND EMAILED TO [email protected]
